Calm in the Moment: Coaching Teams Through Distressed Reactions When Caring for Individuals Living with Dementia
This session explores one of the most urgent challenges in aging services: effectively preventing and responding to distressed reactions among individuals living with dementia. Too often labeled simply as “behaviors,” these expressions are frequently misunderstood. In reality, they are meaningful communication—signals of unmet needs, fear, pain, trauma, boredom, or environmental stress. When care partners lack the knowledge and practical tools to interpret these messages, responses can unintentionally escalate distress, contribute to team member frustration and burnout, and diminish quality of life for those receiving care. Participants will gain a practical, person-centered framework for understanding the root causes of distressed reactions and implementing proactive, non-pharmacological strategies that reduce risk and build trust. Through real-world examples and actionable interventions, this session equips leaders and frontline teams to shift from reaction to prevention—creating calmer environments, stronger relationships, and more compassionate dementia care outcomes across settings.

How I Learned to Stop Worrying and Love Medicaid Eligibility
The successful evaluation of the financial profile of a resident applying to your facility can mean the difference between a resident whose Medicaid eligibility is approved without issue (leaving a revenue stream) or a resident who becomes a financial albatross without MA eligibility (or a significant penalty). This seminar will provide some times/pointers on areas to be aware in the evaluation of the financial background of a resident anticipated to be Medicaid pending and how to ensure that their account does not end up costing you potentially hundreds of thousands of dollars in lost revenue. We will review the eligibility process with an eye towards issues that can create penalties or ineligibility and what red flags to keep an eye out for. We will also discuss recent changes to the law regarding further restrictions on the attachment of property and other ways to ensure consistent revenue stream if problems arise.

Designing amenity areas for seniors is about more than aesthetics—it’s about dignity, independence, and daily comfort. When Substance Use Enters Long-Term Care
Residents do not arrive at long-term care as blank slates. Increasingly, they enter nursing homes and assisted living communities with established patterns of alcohol misuse, prescription dependence, opioid exposure, or diagnosed substance use disorders. National surveillance data from the National Survey on Drug Use and Health and the Centers for Disease Control and Prevention demonstrate rising substance use trends among adults aged sixty-five and older, making these complexities more visible at admission. This session explores what it means when substance use enters the long-term care environment. Drawing from current epidemiologic data, geriatric prescribing guidance, and addiction research, participants will examine admission assessment considerations, behavioral differentiation, medication safety, and coordinated team strategies. The focus is not on blame, but on readiness—equipping leaders and clinicians to respond thoughtfully, safely, and collaboratively when substance use becomes part of the care landscape.

Active Aging of Senior Consumers: Insights and Opportunities
Today’s consumer is an aging adult, typically 65 or older, who is increasingly taking a more active, technology-enabled role in managing their health and wellness. What does this mean for senior living communities? How can we partner with seniors to deliver a model of care that meets their needs?
This session will begin by reviewing the 9 principles of active aging giving examples of how these can be incorporated into all facets of senior living. The session further defines and describes technology innovations and other DTC health services providers can explore as new service lines or use to enhance existing service lines. Evidence-based research and results of key focus groups will be presented to support the discussion. An Evidence-Based Approach to Assessment of Decisional Capacity in Long-Term Care Settings
Assessment of decisional capacity is a delicate and often challenging process. It involves careful consideration of complex factors and potentially conflicting priorities. Unfortunately, the desire to empower is sometimes at odds with the need to protect. With age being the greatest risk factor for dementia, older adults are at a heightened risk for cognitive impairment. They also tend to be faced with complicated end-of-life decisions. As providers and caregivers for long-term care residents, we must remain keenly attuned to their decisional capacity. This presentation outlines all the necessary components of a comprehensive decisional capacity evaluation and provides examples of evidence-based tools to substantiate conclusions. It also offers guidance on timing of capacity referrals and clarification of referral questions. Case examples will be reviewed.

Can Transformative Leadership bring Staff Consistency and Culture Change?
High turnover among clinical and administrative staff poses risks to patient safety and interferes with team cohesion. In many healthcare settings, annual attrition rates exceed industry benchmarks by 15–20%, adding up recruitment costs and disrupting continuity of care (Rošková, Schraggeová, & Jakubek, 2024). Concurrently, caring for older adults with cognitive, medical, and functional challenges requires that patient-centered practices and cultural humility are deeply understood by the entire team. This leadership need assessment is intended to understand the gap between current leadership capacities, leadership styles and the competencies required to stabilize staffing and guide meaningful culture change to improve outcomes for older adults.
Culture surveys suggest teams often work in silos, having an “in-group” and “out-group.” In addition, communication barriers and resistance to change or new equity initiatives often plague health care teams (Jerab & Mabrouk, 2023).

Closing the Gaps in Advance Care Planning: Building Alignment, Access, and Accountability Across Care Settings
Advance care planning (ACP) is a core component of post-acute care. As quality measures tighten to align care with stated preferences, high-quality, interoperable ACP is increasingly in the spotlight. For many acute-care providers, the growing emphasis on care alignment requires renewed attention.
ACP programs frequently don’t operate at their highest potential or achieve their primary objective: helping individuals avoid unwanted or non-beneficial interventions and aligning care more closely with their documented treatment preferences. Too often, documents are inaccessible at critical moments of decision-making. When clinicians cannot reliably retrieve treatment preferences, care teams may default to high-intensity interventions that do not reflect patient wishes. Fragmented processes further increase the risk of delivering care that patients may not have chosen.
This session will present a structured framework and practical tools that organizations can implement to strengthen ACP programs and improve the consistency, accessibility, and reliability of documented treatment preferences across care settings.

Improving Family Members' Engagement and Satisfaction Through Psychoeducation
Having a family member in a long-term care facility can be extremely challenging. Relinquishing control of a loved one's care can elicit a range of emotions from guilt to frustration. If cognitive impairment is a factor, it can be hard for family members to get a clear picture of their loved one's experience. This is especially true if denial is at play. Unfortunately, a family member's frustration with circumstances can sometimes be projected onto the facility, leading to conflict or disengagement. This presentation offers specific strategies for communicating with families, resolving conflict, and educating them about their loved one's condition. Dealing with denial regarding cognitive impairment is highlighted. Specific case examples are offered.

Advancing Safer Chemical Practices: Outcomes, Toolkit Development, and Lessons Learned from a Maryland EPA Grant Initiative
In 2024, the Maryland Department of the Environment (MDE) received a U.S. EPA grant to collaborate with a handful of long-term care facilities in Maryland, to promote the adoption of Safer Choice certified products. Overuse of chemicals and lack of training of environmental services and frontline staff has led to poor indoor air quality, infection prevention challenges, and adverse health impacts for workers and residents living within extended care settings. Building upon the 2025 LifeSpan Conference presentation, which introduced the scope of the grant, including facility audits, stakeholder focus groups, staff and resident training, and policy development, this session presents the program’s final outcomes and deliverables. Attendees will receive an in-depth overview of the completed toolkit, including on-demand training modules, policy and procedure templates, and chemical selection guidance. The session will also feature a comprehensive case study overview, highlighting implementation challenges, stakeholder engagement strategies, measurable impacts, and key lessons learned.

Creating a Talent Pipeline for Your Organization: The Opportunities of Internships, Externships, Mentorships, and Apprenticeships
With the increasing population of older adults, the increasing range and diversity of career paths in this sector, and the increasing need for professionals in these careers, it is important that higher education programs prepare students for these opportunities. Experiential learning opportunities are an effective and valuable way of doing this. However, many employers may have a preconceived idea of internships, externships, mentorships, and apprenticeship programs as highly structured, lengthy, and expensive. Additionally, many may associate apprenticeships with construction and building trades. Such assumptions can prevent organizations from fully exploring the benefits of these experiential learning opportunities to both the student and the host organization. Expectations may be that hosting students in these contexts would be disruptive and too cumbersome. This presentation will demonstrate that these programs can be flexible, scalable, and accessible, and can deliver value to both students and organizations.
