Getting to the Cause of Problematic Behavior In Order to Treat It
Problematic behaviors arise amongst patients with Alzheimer's and Dementia living in community with each other, whether it be with other patients or staff. Understanding the antecedent to the behavior, context, and consequences that occur following the behavior can provide insight into the behavior as a whole. Behavior planning is an effective way to implement change and modify behavior to be more adaptive and functional. To achieve the best outcomes for the patient and all providing care/services it is imperative that all staff are trained in how to utilize the behavioral interventions.This training offers staff the opportunity to make adjustments that will have a lasting impact increasing satisfaction and self-efficacy, while generating the ripple effect of improving the overall wellbeing and quality of care to patients in compliance with state and federal regulations, mitigating risks, and operating a thriving business.

Assisted Living Regulations Update
During this session, the Office of Health Care Quality (OHCQ) will present the history of what it took to get the current new regulations (10.07.14 - Assisted Living) promulgated. The attendees will also see a side by side comparison of the old regulation requirements with the new regulation requirements. The leaders and staff of the assisted living will benefit by using the knowledge they have learned to bring back to their organizations and implement the new requirements to maintain compliance with regulations.

Disparity in Dementia Between African American and White LTC Residents: Enhancing Caregiver' Cultural Awareness and Sensitivity
A growing number of epidemiological studies indicate that African Americans are two to three times more likely than White counterparts to have Alzheimer’s disease and related dementias. This disparity may stem from a complex interaction of racial differences in biopsychosocial and historical factors. The BCAT® Research Center conducted a study of LTC residents to explore the nature of this disparity, looking not just at differences in prevalence of dementia but at age of onset and rate of decline. The primary purpose of this presentation is to describe the results of the LTC study in order to inform and improve caregivers' cultural competence in treating patients with dementia.

Engaging Residents with Dementia: Strategies for Promoting Positive Care Interactions
The goal of this session is to raise awareness of the importance of staff-resident care interactions in long-term care settings. Care interactions are essential to promote quality of life and well-being among residents living with dementia. Currently, care staff have limited knowledge on how to appropriately engage residents living with dementia. Residents living with dementia may have difficulty engaging in care interactions due to severity of dementia, presence of sensory impairments, or other communication difficulties. This session will review barriers to effective, positive care interactions and delineate the factors associated with positive and negative care interactions. Specifically, it will focus on the association between resident factors (e.g., engagement, pain) and the quality of staff-resident care interactions among nursing home residents living with dementia. In addition, the session will provide practical approaches and suggest interventions to increase positive care interactions between staff and residents while eliminating negative and neutral care interactions.

Legislative and Regulatory Update: What’s Hot and Happening with SNFs and ALFs
Regulatory guidance, policy efforts, and legislative priorities regarding skilled nursing facilities and assisted living facilities are constantly evolving. The new Trump Administration is quickly initiating significant policy changes across the board. In addition, states are pursuing important regulatory efforts regarding assisted living facilities. It is essential that those providing services in and to the long-term care community stay up-to-date regarding the new Administration’s priorities and other state and federal developments, which may have an impact on their operations, costs, staffing, and the quality of care they may provide their residents. Longevity Ready Maryland: Transforming Aging in Maryland
Aging has changed significantly in recent years, and how we plan for the future must evolve too. With longer lifespans and a growing population over 60, our state’s multisector plan for aging, Longevity Ready Maryland (LRM), provides a framework to holistically address all of the ways longer lives impact our society.
We will explore LRM’s four Epic Goals and how we apply a longevity lens to build community-wide systems of care that promote healthy aging, strengthen local economies, and improve the quality of life for all residents.
With both challenges and opportunities facing the senior living sector, LRM highlights the need for Maryland to move beyond the traditional approaches to aging services and better address the multifaceted priorities 21st-century Maryland communities are facing.

Post-Fall Huddles in Long-Term Care: Enhancing Safety, Preventing Future Falls, and Reducing Fall Severity
Falls in skilled nursing facilities (SNFs) are a significant concern due to their high prevalence and impact on residents' health and well-being. In addition to the physical and emotional toll falls take on clients and caregivers, falls result in increased hospitalization and health care cost. Despite fall prevention programs, many SNFs struggle with consistent implementation of evidence-based interventions. This session will explore the evidence supporting post-fall huddles in long-term care facilities and their impact on patient safety and quality of care. We will discuss how implementing structured huddles can enhance communication, identify fall risk factors, and reduce both the frequency and severity of falls. Attendees will gain practical strategies for integrating post-fall huddles into their facilities to improve outcomes and foster a culture of safety.

Senior Living Communities are focused on client services and needs. But many do not have menus or options for non-alcohol drinkers. By creating an inclusive, exciting non-alcoholic beverage program, you can attract discerning shoppers, and significantly increase revenue. With the Surgeon General pushing for a warning label indicating alcohol’s inclusion as a Group 1 carcinogen (along with tobacco and processed meats) and market data showing double-digit growth in non-alcoholic beverage sales last year compared to alcohol’s 1%, this is not just a trend. Safer Choice Chemicals: Protecting the Indoor Environment, Residents, and Staff
In 2024, the Maryland Department of the Environment (MDE) was awarded a U.S. EPA grant to work with a handful of long-term care facilities in Maryland, with the intention of encouraging the use of Safer Choice certified product in their facilities. Overuse of chemicals and lack of training of frontline environmental services and maintenance workers has led to poor indoor air quality and adverse health impacts for workers and residents living within extended care settings.
The scope of the grant program includes staff and resident training/education as well as the development of policies and procedures, including procurement, to increase the use of safer and more sustainable chemicals and products for cleaning, disinfection, and maintenance. Upon completion of the grant program, a final toolkit, including training and policy and procedure templates will be developed and made available to the long-term care industry throughout Maryland.

After each musical selection, participants will build responses to the music with LEGO bricks. The collaborative sessions will allow participants to experience the power of both listening to music and creating their own responses to it. My patient had sepsis! What next?
This presentation will focus on the disease state of sepsis, along with the overall financial, business, and patient-level impacts. We will start with a high-level overview of sepsis and will then discuss how to care for sepsis-recovering patients in the post-acute setting. We will examine how a collaborative team was formed with a medical provider group, a skilled nursing facility, and a hospital system to tackle high readmission rates and frequent patient decompensations for patients recovering from sepsis in a post-acute/long-term care facility. The presentation will include the formation of the collaborative group, the process and timeline, implementation, challenges, results, and ongoing expansion of the program.

Preventing Infections, Protecting Lives: Dialysis in Skilled Nursing Facilities
Dialysis in skilled nursing facilities faces unique infection control challenges not seen in outpatient dialysis locations. This presentation will examine the different types of dialysis facilities in skilled nursing facilities, highlighting the key differences to better understand the challenges they may face. This presentation will focus on common infection control gaps observed during onsite infection control assessments within Maryland. This presentation will highlight the unique challenges seen in dialysis performed in den settings as well as emphasize that some infection control responsibilities overlap between the dialysis and the skilled nursing facility. To address these concerns, best practices for mitigating these infection control gaps will be discussed so skilled nursing facilities can learn how to help prevent infections including the spread of multidrug resistant organisms.

Successful Discharge Begins on Admission – Improve Outcomes through Effective Care Navigation and Live Analytics
According to a 2022 JAMA Network study, 1 in 5 Medicare beneficiaries discharged from the hospital receive care in a Skilled Nursing Facility (SNF). Care transitions are vulnerable moments where complications can lead to readmissions, making effective discharge planning crucial. This session explores a patient-centered, interdisciplinary approach to care transitions, emphasizing the role of care managers and social workers in leveraging post-acute analytics and Social Determinants of Health (SDoH) to anticipate patient needs. Attendees will learn strategies to enhance care coordination, facilitate effective communication across all stakeholders, and implement best practices that reduce Emergency Department utilization and rehospitalizations. We will also highlight care management practices, including transition of care standards, that improve outcomes for high-risk patients.

Tardive Dyskinesia in Long Term Care
Tardive Dyskinesia (TD) is a Distinct Movement Disorder Associated with Prolonged Exposure to Dopamine Receptor Blocking Agents (DRBAs). Differential diagnosis of drug induced movement disorders Is necessary for appropriate monitoring and treatment. Use of multiple treatment options may be necessary in patients with multiple DRBA-induced movement disorders. Older age residents on antipsychotics are at higher risk for tardive dyskinesia. TD diagnosis has been shown to be associated with higher all-cause healthcare costs. TD can also affect the QOL in LTC including effects on physical functioning and ADLs as well as psychosocial functioning. This presentation will focus on the appropriate screening, diagnosis and treatment of TD as well as the burden associated with this debilitating condition.

Why We Sing: How Communal Singing Support Healthy and Creative Aging
Dr. Gene Cohen’s groundbreaking Creative Aging study, The Impact of Professional Conducted Cultural Programs on the Physical Health, Mental Health, and Social Functioning of Older Adults-2-Year Results (2007) inspired the founding of Encore Creativity for Older Adults. Study participants engaging in weekly choral rehearsals for two years experienced many improvements to their health and wellbeing including decrease in falls, decrease in medication, and an increase in participation in other community activities.
Since that study, many others have been conducted to explore the role music (especially singing) plays in health and wellness as we age. This session will present an overview of this work and how it can be used in conjunction with Hettler’s Wellness Wheel to design meaningful choral experiences for older adults.
Session attendees will have the opportunity to experience some of the music activities that Encore incorporates into our Arts for Life programming for people with memory loss.

How Social Determinants of Health Impact the Health of Your Workforce
A large proportion of those employed in the senior living industry earn what the Brookings Institute describes as a lower wage. This interactive panel discussion will introduce the Social Determinants of Health (SDOH); how financial and other resource insecurities impact those earning a lower wage; how to use data to explore the impacts of SDOH; and strategies to mitigate resource scarcity.
While SDOH impact over 80% of a person's health, until recently, SDOH data remained absent from a person's medical claims. The panelists will discuss how their organization marries publicly available SDOH data with individual member medical claims to uncover the impact of resource insecurity on the health and well-being of team members.

Older Adults' Vulnerability to Scams: Using Knowledge of Psychology and Cognition to Identify Protective Strategies
Older adults are increasingly vulnerable to scams that can leave them in financial ruin. According to the FBI's Internet Crime Complaint Center, roughly $3.4 billion in total fraud losses were reported by people over age 60 in 2023, up 11% from 2022. There are psychological and cognitive variables unique to generation and age group that contribute to increased vulnerability. We will explore these variables and use that knowledge to identify strategies to help protect older adults and prevent fraud.

Top 10 deficiencies and tips to mitigate
CMS is in the process of decreasing the number of outstanding surveys throughout the country and are focused on returning to a sense of normalcy. Infection control deficiencies continue to be cited across the nation at all levels of scope and severity but other areas such as accidents, quality of care and care planning continue in the top ten deficiencies. Civil money penalties are being implemented based on survey findings and have increased in frequency and severity.
Revisions to the surveyor process are soon to be put into place to streamline survey but has focused on improving surveyor knowledge on area related to staff and PBJ. Please join us in discussing the current deficiencies, to identify causes and methods to prevent their occurrence
