Empowering Seniors And Those Who Care For Them Using LEGO® Serious Play®
LEGO® Serious Play® is a methodology created by The LEGO® Group designed to allow participants to use their hands and LEGO bricks to create strategies and solutions. Loretta Veney is a certified LEGO Serious Play facilitator and began using it with her Mom with dementia and hundreds of others in 2014 to uncover long-forgotten memories and to build joy, hope and resilience. Though it was primarily designed to help corporate organizations, Loretta found it to be incredibly engaging and empowering for seniors, including those with dementia and their caregivers! In this session attendees will not only learn what LEGO Serious Play is and how impactful it can be, but will experience how empowering a methodology it is for themselves by completing two short LEGO building activities.

Thrive Under Fire: Handling Difficult Customer Situations Like A Pro
Customer service is the cornerstone of every business and is essential for a strong reputation. We learn about it, we talk about … but do we deliver what we say? In this session, the participant will begin by completing a brief self-scored inventory to identify the personality traits that apply to them. This trait score will be utilized to facilitate a better understanding of themselves and how they best relate with others (of similar or different personalities). Because we each encounter “difficult” customers (staff, residents and families) in our daily work, the session will address the 10 different “difficult” personalities and offer specific skills and strategies to address each type so to bring out the best in people (even at their worst). Participants will be offered real-time strategies they can utilize in their daily work to heighten awareness of customer service with the end goal of improving satisfaction and outcomes.

A Roadmap To Empowering Your Resident Through The Resident Assessment Instrument (RAI)
The RAI System is your map that guides you to look at the resident holistically. Resident assessments and interviews are the first step in your journey to assessing the resident’s care needs in a long-term care facility or Skilled Nursing Facilities (SNF) per the resident’s “voice.” The Minimum Data Set (MDS), Care Area Assessments (CAAs) and person-centered care plan creates a path for the Interdisciplinary team (IDT) to navigate the resident’s individual care needs, preferences, goals, and interventions.
Residents should be encouraged and supported to make decisions about their care and develop their own goals while supporting staff to identify and carry out interventions. The IDT members can assist the residents and their family members with achieving the desired goals and outcomes.
Take a journey with us as we look at key areas of person-centered care using the RAI system.

How Closed Circuit-TV (CCTV) Will Improve The Care You Provide For The Elderly
Integrating CCTV into your services can significantly enhance the quality of care and operations for elder care. CCTV is not a replacement for direct personal attention, however, it should be an essential component to providing optimal care for the elderly. The balance between navigating privacy concerns and ensuring optimal care for fragile and vulnerable individuals is possible. Arbor Place, a residential dementia community, has used CCTV throughout its community for 22 years, demonstrating that privacy versus safety does not need to be an "either-or" choice. When implemented correctly, families, community members, and staff appreciate the added safety, comfort, and rapid response CCTV supports. Walter Fanburg, MD, founder and geriatric psychiatrist, shares proven, practical approaches and measures on incorporating CCTV to improve clinical outcomes, ways to add CCTV to Qualty Assurance Care Plans, and Performance Improvement (QAPI) programming. He will also share guidance on implementation and how to address barriers to help ensure uncompromising, optimal care care for those entrusted to you.

Paving The Way For A Longevity Ready Maryland
People are living longer than ever before, with Marylanders age 60+ expected to represent more than a quarter of the population by 2030. Older adults offer tremendous value to society, fueling local economies, providing support for families, and contributing in many ways. To maximize the economic and social benefits of an aging society, the Maryland Department of Aging and Governor Wes Moore recognize the need to update policies, systems, and services that address the challenges and opportunities ahead. Our state's multisector plan for aging - known as Longevity Ready Maryland - is a roadmap to coordinate services, increase collaborations, and encourage partnerships among aging organizations that will lead to healthier and more purposeful longevity for all. We have begun planning, developing policies, supporting legislation, advocating, and forming cross-sector collaborations that serve older adults, families, and caregivers and expect to reach more milestones and achievements in the years to come.

What Matters Most For Staff And Residents: Building Organizational Capacity For Providing Quality Long-Term Care
This presentation will describe three evidence-based practices that can be implemented in long-term care (LTC) settings and improve outcomes for both staff and residents. (1) Increasing capacity for palliative care: Older adults with serious illness lack palliative trained clinicians in LTC settings. We will describe a project to build internal capacity through education and training, specialist support, and criteria to identify LTC residents who most benefit from services. (2) Optimizing meaningful activity: Meaningful activity is beneficial for persons with dementia, but limited opportunities exist for LTC residents with dementia to engage in meaningful activity. We will describe a theory-based approach and practical tools to implement in LTC settings. (3) Nurturing staff wellbeing: LTC settings are experiencing an unprecedented staffing crisis, which can have an impact on workers’ health and resident outcomes. We will share lessons learned and strategies to provide practical solutions to address staff wellbeing in LTC settings.

Improving Pain Assessment, Diagnosis, And Management By Implementing The Pain Management Clinical Practice Guideline (CPG)
Pain is common among older adults, with 30 to 80% of those in nursing homes experiencing pain. For over 50% of individuals the pain is moderate to severe. Pain often results in a decline in function and quality of life, exacerbates behavioral symptoms associated with dementia, contributes to delirium, and increases emergency room visits. Unfortunately, there are staff, resident and family challenges to assessment, diagnosis and management of pain. Regardless, the State Operations Manual for Long Term Care, specifically F-Tag 697, requires that nursing homes must ensure that pain management is provided to residents who require such services. This session will provide an overview of the new Pain Management Clinical Practice Guideline (CPG) for nursing home residents and provide the simple four step approach for how to implement the use of this guideline in real world settings. Access to the guideline will be provided without cost to participants.

Managing Sepsis: Insights, Analytics, And Strategies For Nursing Facilities
The burden of sepsis is high in the U.S., with over 1.7 million adult cases resulting in 350,000 deaths reported annually. This session explores how nursing facilities can leverage live data analytics to detect the risk of sepsis early, allowing clinicians to intervene in care prior to an adverse event or need for hospitalization. Learn how Maryland’s designated Health Information Exchange (HIE), the Chesapeake Regional Information System for our Patients (CRISP), is helping facilities within the program reduce sepsis-related cases. By utilizing live alerts pulled from data that already exists in the resident’s EHR, care teams are identifying the early onset of sepsis and providing interventional treatment before readmission is necessary. We’ll also discuss how CRISP tracks cases, interventions, and antibiotic alerts in nursing homes, and share strategies and tools facilities are implementing to help manage sepsis among their patient populations.

Palliative Care As A Tool To UPLIFT The Care Of Nursing Home Residents With Dementia
Palliative care is an effective model of care focused on maximizing quality of life and relief from suffering for people with serious illnesses, including dementia. Evidence shows that many nursing home residents are eligible for and would benefit from palliative care services. Yet, palliative care is not consistently available in nursing home settings. There is a need to test pragmatic strategies to implement palliative care programs in nursing homes. Drawing on findings from UPLIFT (Utilizing Palliative Leaders in Facilities to Transform care), a large multi-state clinical trial involving sixteen nursing homes, this session will discuss ways to bring research and care to both the long-term care and skilled nursing center. Attendees will learn ways to strategically partner with the community, while building a sustainable palliative program designed to improve quality of life and quality of care for residents with dementia and their families.

Panel Discussion On Advanced Directives: The What, The Why, And The How To (And Who Can Help)
Advance care planning and advance directives provide guidance during a time that is often characterized by stress and heightened emotions, and ensure that the wishes of the individual are known when they may not be able to speak for themselves. Despite their value, fewer than half of adults have a legally binding plan in place. Join this discussion to learn what an advance care plan provides, how to partner with diverse professionals to inform your residents (and staff!) about the benefits of planning, and discover the options available for creating, storing, accessing, and sharing plans once developed.

Public Health Partnership To Long-Term Care Setting
The Howard County Health Department (HCHD) Infectious Disease Surveillance and Response (IDSR) program developed an in-person site visit initiative to help address the need for our nurses to effectively get out into the community to distribute information regarding infectious diseases, reporting requirements, and to build relationships with long-term care (LTC) facilities. IDSR nurses challenged ourselves to visit every LTC facility in Howard County within a year, AND WE DID! In our presentation the IDSR nurses will describe our program and partnerships with LTC facilities in our jurisdiction. Attendees will gain knowledge of tools that can be utilized by LTC facilities and to collaborate with the health department to address infectious diseases.

The Unsung Heroes Of Rehabilitation And Nursing Homes: Recognizing The Vital Role Of Registered Dietitian Nutritionists
Malnutrition is common among elderly patients in healthcare settings such as hospitals, rehabilitation centers, and long-term care facilities. Patients may experience insufficient food consumption, weight loss, and deteriorating health. Furthermore, these individuals are more susceptible to developing wounds, dehydration, and other nutritional concerns, which can contribute to a greater likelihood of declining health and an extended stay in rehabilitation and nursing home facilities. Registered Dietitian Nutritionists play a crucial role in delivering medical and nutritional therapy (MNT) interventions to enhance the overall well-being of these patients.

Implicit Bias And Micro-Aggression: Understanding How Well Meaning People Can Cause Harm Through Everyday Interactions
Sometimes the hardest person to face is YOURSELF and your belief's. You will not believe well meaning people do and say harmful things to their colleagues everyday. Well they do and YOU do! It does not mean it was intentional but YOU do. This interactive and informal setting will help participants be more acutely aware of the every day encounters in the Aging Services workplace and the common language and behaviors that undermine productive and inclusive cultures. Participants will unearth and see themselves and better understand their behaviors and patterns ,as well as, that of others in transforming their organizations cultures toward thriving best practice filled diversity, equity, and inclusion cultures. Self awareness is our greatest tool. If you believe everyone matters, then be willing to be vulnerable, and have the courage to face YOU.

An Approach To Trauma-Informed Care Designed Specifically For Older Adults
Recent legislation requires facilities to ensure that residents “receive culturally competent, trauma-informed care in accordance with professional standards of practice and accounting for residents’ experience and preferences.” However, most trauma-informed care approaches do not adequately recognize generational differences in coping styles or cognitive changes that affect how a person processes and responds to triggering events. We explore the effects of chronic stress on the brain, the lifelong impact of trauma, and the link between trauma and dementia. We also describe an approach to trauma-informed care designed specifically for older adults. The approach gives suggestions for screening and assessment, as well as guidance for promotion of holistic resilience in residents and the creation of a safe physical and social-emotional environment.

Application Of An Evidence-Based Tool Designed To Measure Situational Awareness For Management Of Dementia-Related Behaviors During Personal Care
This presentation outlines the development of a new tool designed to measure the construct of situational awareness (SA). SA refers to a person’s perceptual recognition and cognitive understanding of an immediate environmental context (e.g., bathing, toileting, dressing, participating in a life enrichment activity). SA contains four components: orientation, command processing, verbal engagement, and visual attention. Patients with moderate to advanced stage dementia exhibit a decline in SA, which can be associated with dementia-related behaviors, resistance to care, and falls. In the presentation, we describe how the evidence-based SA tool can be implemented by various disciplines, especially nursing and rehab, what to expect from patients within each score range, and strategies for managing dementia-related behaviors from patients with specific score profiles. We showcase the easy-to-use crosswalk table that comes with the tool, which highlights these strategies.

Planning For Construction & Maintenance Activities In Healthcare Facilities
Construction and renovation activities in healthcare settings require planning and comprehensive strategies to safeguard occupants. Originally integrated into CDC's Guidelines for Environmental Infection Control in Health-care Facilities, Infection Control Risk Assessment (ICRA) procedures have evolved alongside technological advancements and enhanced work practices to minimize the risk of infections stemming from construction and maintenance work.
In October 2021, the American Society of Healthcare Engineers (ASHE) introduced the update ICRA 2.0, intended to enhance and clarify the current ICRA process, which is focused on project activity types and patient risk group in order to determine the level of precautions needed. Notably, ICRA 2.0 expands its reach into all types of healthcare facilities, including LTC, and introduces a new fifth class of precaution, Class V, specifically tailored for extensive projects. This presentation will provide an overview of ICRA 2.0 and innovative industry solutions designed to streamline compliance and facility integration.

Sink Or Swim? How To Succeed In A Value-Based Care World
As CMS aims to have all Medicare beneficiaries and most Medicaid beneficiaries enrolled in a value-based care program by 2030, it is imperative for nursing facilities to begin developing innovative strategies to adopt value-based care initiatives that will improve facility performance, occupancy, and revenues. During this presentation, experts will discuss the various types of value-based programs available to facilities and proven strategies to improve facility performance. Session speakers will also share best practices on how accessing live data analytics, pulled directly from the facility EHR, can reduce clinician burnout and administrative burdens by prioritizing clinical care, improving quality outcomes, lowering avoidable readmissions, and managing length of stay. Improving these key metrics enables facilities to demonstrate value as high-performing providers for their hospital and health plan partners thereby increasing referrals, boosting revenues, and succeeding in value-based care initiatives.

Dementia Stigmas And Spiritual Therapy
Spirituality is personal, some link it to religious rites, while others connect with a higher power through nature and art. As a coping resource, spirituality can support persons living with dementia in sustaining meaningful relationships, and purpose while drawing strength from comfort and hope. Dementia is accompanied by stigmas; the focus becomes what the person can no longer do instead of what they can still do. Caring for their spiritual health and wellness can help the person with dementia live a more meaningful and satisfying life. In the care setting, benefits would include making sense of situations, finding purpose, framing the past, feelings of approval, feeling valued, and lessening the fear of losing self. It will take effort, practice and mindfulness to successfully enrich the lives of this vulnerable population, join me in nurturing others and you will feel nurtured.

Don''t Fight The Fed: How Recent Action By The Federal Reserve Has Impacted Seniors Housing
The Federal Reserve, with its power to control money supply in the USA, was expected to cut rates in the early months of 2024 to curb the effects of inflation. With inflation still being 3% higher than the target, the Fed is not likely to cut rates anytime soon. As a result, the cost of capital remains historically high - making it more difficult for operators, developers, and investors to obtain the financing necessary to flourish. What are they going to do next? When can we expect rate cuts? Who is going to figure out how to grow in this high-rate environment to take care of our seniors? We will tackle these questions and gain insight that can help you manage your future capital decisions.

Empowering The Future Of Healthcare Through Impactful Recognition Programs
A tight labor market, together with high turnover, has led to staffing shortages in healthcare communities across the country – impacting their ability to recruit and retain healthcare workers and provide quality care patients and residents deserve.
The need for low-cost, high-impact initiatives to increase worker resilience, improve retention, and maintain high safety and quality of care standards, is greater than ever. Workforce recognition impacts them all and is a strategy Harvard Business Review has cited for its oversized impact on healthcare workers.
But impactful recognition has certain key characteristics. And the “science of recognition” can help organizations and leadership teams effectively reinforce behaviors that contribute to an exceptional patient experience.
Lifespan members will share their experience with recognition programs, and how they can empower the future of aging services.

How I Learned To Stop Worrying And Love The Involuntary Discharge Process - An Overview Of The Current Landscape
Involuntary discharges are not dead. Despite significant changes in 2019 to the process (including many changes to the post-discharge planning process, the content of the letters and the identification of discharge locations), the involuntary discharge process remains an effective tool to effectuate the removal/discharge of non-paying or behaviorally problematic residents. While challenges remain that beg legislative solutions, the availability of mediation and the leverage of an Order of Discharge remain a powerful tool to effectuate the removal of those residents who refuse to pay or whose behavior can itself become a liability. Understanding the process and how to fully comply with the provisions of 19-344 and the Surveyor Guidelines can help problem-solve and keep you away from an adverse survey.

Best Practice Guidelines For The Management Of Indwelling Catheters And Treatment Of CAUTIs
It is estimated that US healthcare system spends $400-500 million on CAUTI prevention a year. While there is some evidence to support certain catheter management and infection control measures, clinical practice may vary widely. In this session we will cover best practice guidelines for catheter selection, catheter care and explore interventions for common catheter related complications. Accurate diagnosis of CAUTIs is also challenging and we will review NHSN criteria for identification and treatment.
