Age Positivity and the Case For Artistic Activism in Elder Care
Research now shows us that having positive age beliefs can extend one''s lifespan as well as reduce the risks of developing a host of diseases, including dementia. Damaging stereotypes about being older are difficult to eliminate in our society, not to mention within the walls and campuses of senior living settings. The good news is that there are time-worn and effective tools that can help to dismantle age negativity. In this highly interactive session, participants will learn about aging services providers that are utilizing art and activism to tackle damaging stereotypes about older adults while also bringing age positivity to both residents and staff. Participants will learn how the arts and creative expression can be deployed to transform communities into becoming change agents for in the movement to end ageism, improve quality of life for elders, and boost age positive beliefs.

Are You Communicating ... or Connecting?
In a world where there is a lot of noise, plenty of talking, and apparent communication we can often mistake information dissemination and knowledge transfer with actual connection...but the two are not always synonymous. The word "communication" actually has a common derivation with the words "community" and "common ground." Too many of us think we have communicated with others only to be miles apart and share no common ground, and there is no real community. This can take place in every context of life, and certainly in the professional leadership context. Everything we do as leaders in creating value is done in and through people. To maximize effectiveness and success, therefore, communication must involve connection. Leadership is influence, and to influence one must connect. Knowledge and information transfer alone is not enough in leadership.

Compassion Fatigue: Adaptive Coping Skills for Healthcare
Compassion fatigue is a condition characterized by emotional and physical exhaustion leading to a diminished ability to empathize or feel compassion for others, often described as the negative cost of caring. It is sometimes referred to as secondary traumatic stress. Compassion fatigue is considered to be the result of working directly with victims of disasters, trauma, or illness, especially in the health care industry. People who experience compassion fatigue may exhibit a variety of symptoms including lowered concentration, numbness or feelings of helplessness, irritability, lack of self-satisfaction, withdrawal, aches and pains, or work absenteeism. In this session you will learn adaptive coping skills that can counter compassion fatigue in the senior care setting.

Diagnostic Testing in Post Acute Care - The Future is Now
In a fun and interactive session, participants will learn about the testing options that will both get results back sooner and be even more accurate.Molecular based testing has been around for years and was brought to the forefront during the pandemic. Utilizing lessons learned and knowledge gained, clinical staff in congregate style living facilities have new options to choose from in order to provide more timely and accurate care to their patients. New molecular-based testing - combined with specimen obtainment modalities - decreases the need for invasive procedures. Future improved patient outcomes will be determined by more accurate tests and the ability to treat patients sooner. These actions lead to a decrease in the need for hospital transfers. Once a facility embraces the power of these new diagnostic tools, quality outcomes and antimicrobial stewardship both improve.

Director and Officer Issues in Healthcare: What Every Board Member and Executive Should Know to Avoid Personal Liability
Knowledge of duties and protections for executives of organizations is critical in the highly-regulated world of healthcare. This presentation uses the adoption of a firearms policy and COVID protocol as two case studies to examine the fiduciary duties that executives owe to their senior living organizations, and in some instances to third parties. We will review what those duties mean, how they typically present themselves in managing organizations, and how directors and officers can properly exercise the duties they owe. After identifying the obligations, risks, and exposure, the presentation will discuss best practices in governance decision making, interacting with employees, and protections available to guard against personal risk to healthcare executives.

Don’t Be Decieved: Pressure Injuries vs. Incontinence Associated Dermatitis, and Why Clinicians Need to Know the Difference
In every health care setting, CMS is tracking the number of pressure injuries which are new or worsening. Pressure injuries are costly -both in treatment and legal costs. However, many “pressure injuries” may be misdiagnosed. Incontinence-associated dermatitis can often mimic a pressure injury, and knowing how to differentiate these two conditions will help lower legal risk, improve outcome measures, and improve patient satisfaction.

How Caregiver Recognition Can Improve Employee Resilience and Retention
Perhaps the most pressing threat to quality healthcare today is staffing shortages. These shortages force skilled nursing facilities to deny admissions which impact occupancy rates and bottom lines. Across care settings, workers left during "The Great Resignation" and they aren’t coming back. This situation arose during the pandemic and has been exacerbated by social and generational shifts. The need for low-cost, high-impact initiatives to increase worker resilience, reduce burnout, improve retention, and maintain high safety and quality of care standards is greater than ever. Enter workforce recognition, a strategy that Harvard Business Review has cited for its positive impact on healthcare workers.

How Virtual Reality Can Revolutionize Senior Wellness
Virtual reality (VR) immerses a user into an alternative environment to give a sense of presence. The immersive experience stimulates cognition and emotion in ways that TV or movies cannot achieve. A study performed by MIT (2018) found that almost 39% of a group of assisted-living residents reported “better perceived overall health” after watching VR images related to travel and relaxation. For another group who watched the same images on TV, the figure was only 14.3%. Another study concluded that VR is a positive approach to treating memory deficits. This presentation will outline the researched-backed benefits of implementing virtual reality for senior care, share practical strategies to launch a VR program, and provide an opportunity for participants to experience VR.

Integrating Best Practices for Employee Wellness through Health Behavior Change
Employers may think stress and pressure push employees to perform better and faster, but what they fail to recognize are the hidden costs of poor health and a disengaged workforce. Organizations from different industries offer wellness programs for their employees, but few programs offer stress reducing benefits that are not a burden to the organization. Employees who are experiencing burnout, sustained levels of stress, and lack of engagement may need additional support to initiate behavior change. Health coaching is a proven method for helping employee’s lower workplace stress, thus increasing productivity and overall job satisfaction. Health coaching is a partnership between the employee and the health coach, where they work collaboratively to initiate and sustain health behavior change. Investing in your employees' well-being will not only bolster retention and help to attract top talent, but it will also improve your financial bottom line.

Key Compliance Considerations in Contracts with Outside Providers and Professionals
In this seminar, we will review key compliance considerations at play in agreements with third-party providers and physicians. We will discuss Medicare rules relating to rate-setting for purchased services. We will also discuss prohibitions on patient “swapping” arrangements, the importance of fair market value and documentation in agreements for administrative services, and understanding when a HIPAA-compliant Business Associate Agreement may be required. Finally, we will review real-world examples of providers who "got it right" and others who "got it wrong."

Medical Power of Attorney and How to Have a Better Death
Many residents have a healthcare power of attorney or advanced directive. However, those forms should just be the beginning of the residents’ decisions about healthcare planning. The difficult decisions usually do not come at the very end but rather six months or a year before, when the doctor says to the patient you have just a few months to live. Prior to that time, residents/patients need to consider in the calm of day how they will react, how aggressively will they want to fight a diagnosis, and when is it time to call it a day.

The Unfortunate Intersection of Depression, Wound Care, and Chronic Disease
Depression, along with other mental health challenges, are common complications of chronic illness and chronic wounds. But in most cases goes undiagnosed and subsequently untreated. It is estimated that more than one-third of individuals with a serious medical condition have symptoms of depression. As healthcare providers in a skilled nursing facility, setting connecting the dots and learning how to assess for early signs of mental health challenges is paramount to creating a better quality of life for residents.

Claiming a Stake in Value-Based Care: I-SNPs Offer a Unique Opportunity
Value-based care (VBC) and bundled payment models have gained significant traction across healthcare settings, with payers rewarding quality over quantity to improve total health outcomes and reduce spending. Since post-acute care represents one of the most costly and challenging segments of the care continuum, ACOs, hospitals, and health systems are aligning acute and post-acute VBC initiatives to deliver better outcomes. Now, skilled nursing facilities (SNFs) and others are increasingly seeking their own opportunities to operate value-based programs.

If there’s one thing skilled nursing facility administrators have come to accept, it’s that COVID has forever changed long term healthcare. With inflation and supply chain challenges added to the mix, long term healthcare facilities are seeing a very different financial landscape than just a few years ago. Using data compiled from Maryland skilled nursing facilities, we’ll discuss the change in industry benchmarks and explore whether we can expect a leveling off going into 2023.Using virtual polling, we’ll gather real-time input from audience members. As part of this interactive opportunity, the audience will get a collective sense of how their ideas about the future of the skilled nursing industry compares to their peers. BONUS – everyone who attends this session will receive a detailed written summary of the annual benchmark report.

How Innovative Technology Can Improve the Employee Experience
High quality direct-care workers are essential to senior living organizations, but given their many employment options at similar salary points they can be difficult to attract and retain. Innovative technology can help senior living organizations become more efficient, do more with less, address staffing challenges, and recruit, hire and onboard team members. Senior living leaders need to think about how to attract and retain employees by considering all the points along the employee experience path where prospective and current employees will engage with technology. The pandemic forced leaders to fast-track technology adoption to address their needs. Now, more than ever, implementing innovative technology solutions and streamlining business processes can help organizations be more efficient and improve the employee experience.

Practical Skills for Purposeful Communications and De-escalation
It is estimated that 20% of people aged 55 years or older experience some type of mental health concern and up to 90% of nursing home residents with dementia exhibit physical or verbal aggression or agitation. These behavioral symptoms present significant challenges for staff, family and peers living and working in nursing homes, assisted living communities, and affordable housing units. Pharmacological interventions do not offer an immediate solution to emergent situations, are not always available, and their use is often associated with significant side effects in older adults. Their behavior often depends upon how they are being treated, their perception of their care, and the severity of their symptoms. This session will focus on practical and immediately applicable strategies that are designed to reduce the likelihood of challenging and dangerous behaviors from ever emerging and includes skills such as purposeful communication, de-escalation, distraction and establishing safe negotiation zones.

The Economic Crisis In Nursing - And How Staffing Mandates May Impact The Situation
We know the COVID-19 pandemic historical impact on our Maryland and nation’s nursing homes. Using analysis of the most recent reimbursement, Five-Star, PBJ Staffing, and Covid-filings data, CLA (CliftonLarsonAllen LLP) will look forward to 2022-close and 2023 to highlight the cost of care and other operations in Maryland and national nursing homes. The results will be shared for Maryland and regions within Maryland. Without further government programs, the result will be a cost of care still far exceeding Maryland and Federal reimbursement rates, projecting negative margins even for the highly-rated, high occupancy. The presentation will close with projecting the financial, workforce, and occupancy impact if/when other states (eg, New York) or a Federal yet-to-be-announced nursing staffing mandates come to Maryland. The unprecedented challenges will likely continue to have financial impacts and result in reduced access to care for seniors in Maryland.

What Are the Current Trends Impacting Long-Term Care?
Even before the COVID-19 pandemic, many changes were occurring in long-term care that were creating staffing and management challenges. COVID-19 compounded this already difficult situation. Today, changing staff attitudes, the Great Resignation, and the presence of a five-generation workforce are among the many trends impacting how we do business. In this program, we will look at the these and other trends and explore how they will create both challenges and opportunities while potentially changing how we do business.

What To Do When Grandma or Grandpa is a Bully
According to the federal government, bullying includes the core elements of unwanted aggressive behavior; observed or perceived power imbalance; and a repetition or likelihood of repetition of behaviors. Senior-to-senior aggression in communal living settings is typically verbal abuse, with men and women being both victims and aggressors. Do you have this behavior occurring in your facility? Have you trained your staff how to manage resident-to-resident issue? This session will identify types of resident-to-resident bullying, and discuss needed procedures and methods to manage the issue.

Communicating with Families: Strategies for Building Beneficial Family Partnerships
The bridge between formal and informal caregivers can be fraught with risk. Accountability and expectations must be clearly and kindly defined. As aging continues, formal and informal caregivers must develop a reliable cadence to track and discuss changes and how those changes are reviewed, with whom, and who has a voice in the care planning. The complexity of family dynamics can cause conflict; as the formal caregivers it is up to us to help families navigate the aging process by designing a Care Communication System that streamlines communications, highlights accountability, and builds a partnership of respect among all caregivers involved in a case.

Durable Medical Equipment - The Gift of Mobility
The Durable Medical Equipment (DME) Reuse Program has successfully assisted Marylanders by improving mobility, increasing independence, and enhancing overall quality of life. New and gently used medical equipment donations are accepted at strategic drop off and pick up sites across the state. Once collected, each piece of equipment is repaired, sanitized, and inspected before being distributed for free to those in need. The DME program also offers training and practice for those using the equipment. Furthermore, the DME program has created new environmental benefits by reducing unnecessary landfill waste. The Maryland Department of Aging and the Secretary of Aging believe the success of the program has been possible, in part, due to the support of Lifespan members, with the expectation that the DME program will continue to expand across the state.

Let's Focus on Nutritional Needs In Assisted Living
Nutrition plays a vital role in maintaining good health and preventing disease, and is more complex in older adults with multiple problems such as drug therapy, dental issues, chronic disease, etc. The literature supports that more complex needs can be optimally met with a nursing team, the resident''s healthcare provider, the resident, a dietician when indicated, and the family/responsible party. Using guidelines such as weight loss, weight gain, and pressure injury guidelines, appropriate actions can enhance a resident''s nutritional status based on their clinical needs.

Engaging With Older Adults: Blending Arts and Sciences For Improved Outcomes
As we care for older adults we often narrow our focus to the medical aspects of their treatment: what pill to give to fix this or prevent that. But are we addressing their well being? Are their alternative or complimentary things that can be offered which might care for the WHOLE individual - not just their diagnosis? This panel discussion will explore ways that the arts and sciences can be utilized to improve outcomes for seniors, leading to satisfied residents, staff and families.

An Evidence-Based Approach to Dementia-Related Behavior Management
This presentation begins by setting the stage for the need for non-pharmacological approaches to manage dementia-related behavior problems. Behavioral and psychological symptoms of dementia (BPSD), health risks and side effects of prescribing psychotropics and antipsychotics to older adults, Centers for Medicare and Medicaid Services (CMS) guidelines governing prescription of these types of medications, and dementia-related medication compliance issues are all reviewed. Next, evidence-based BCAT® (Brief Cognitive Assessment Tool) tools designed to assess cognition and behavior are introduced, with a focus on how these tools can be applied to a dynamic, new behavior management approach. The presentation concludes with case examples showing how the approach can be utilized to manage difficult behaviors such as combativeness during care and inappropriate sexual behavior.

Annual MedicaidMedicare Reimbursement Update for 2022
It has been a few years since Maryland Medicaid rebased (recalculated) its rate setting system, COVID has had a significant change on operator cost and census, and Maryland has had more change of ownership in SNFs than ever before. All of these change the cost reports which are used to determine rates. The 2023 State budget awarded Maryland Medicaid SNF providers an 8% increase - the largest we have had in years, maybe ever. The PDPM system has been in place for a few years now and the numbers are in. What do all these factors combined mean? What does that means for Medicare rates? Are you ready...or what do you need to do to be prepared?

Complying with Maryland’s New Employment Laws
Maryland's General Assembly recently passed a number of employment bills that will significantly impact the workplace. We will explain the obligations of and provide guidance on employer compliance with these new laws. Specifically, we will discuss the creation of a paid family and medical leave program applicable to all employers with Maryland employees, which provides up to 24 weeks of paid leave to eligible employees. In addition, we will cover the expansions to the State anti-discrimination law, including an amplified definition of illegal harassment, an extension of the statute of limitations for employment discrimination and harassment claims, and new(?) reasonable accommodations for applicants with disabilities. We will further consider the workplace impact of the likely legalization of recreational marijuana, and address the revisions to Maryland’s Personal Information Protection law.

Consider the Jury: Learning from Litigation
Litigation has long been one of the greatest challenges to the LTPAC/senior care community, and in the aftermath of the COVID-19 concerns about lawsuits are perhaps greater than ever. Simply providing good care is not enough to protect your facility from becoming the target of a lawsuit, you must also "consider the jury." By reviewing real-life examples of deposition testimony, legal pleadings, and patient charts from actual Maryland lawsuits, you will see how Plaintiffs exploit the public's prejudices and preconceptions to influence the way a jury looks at care. You will also learn the tips and tricks you need to defend yourself, your team, and your facility from liability by getting the jury on your side!

Health Care Bullying – Could This Be A Reason Staff Are Leaving?
According to the Joint Commission, the Workplace Bullying Institute estimates that 65.6 million U.S. workers have been directly impacted by or have witnessed bullying. The nursing staff crisis has been a catalyst to create a focus on why nurses CNAs, or other health care workers have become unhappy with their work environments, suffered burn out, or have chosen to leave the health care field. Increased concern that workforce bullying may be a causative factor requires the industry to recognize, understand, and address this issue. This session will define workplace bullying as it relates to healthcare staff, identify causes, and will address interventions to help in address the issue.

Involuntary Discharges in the Post-HB 592 and Post-COVID World
When HB 592 was enacted into law in October, 2019, few could have predicted that four short months later we would be in the midst of a massive global COVID-19 pandemic. Many significant changes to the involuntary discharge process were enacted and we have now had a chance to see and evaluate them in the current, largely virtual, ID mediation and hearing process. This program will provide a refresher on the ID process overall, the HB 592 changes, and provide a scaffold for all steps you can take including the importance of the post-discharge care planning process and observations of the process. Strategies to maximize success in the process will be discussed in detail, as well as practical challenges (i.e. how practical solutions for discharge of a difficult resident).

Retirement Communities as Creative Hubs
As the industry faces declining census numbers due to COVID-19, this presentation urges retirement communities to reposition themselves as creative hubs and in the process make communal living a more welcoming environment. Focusing on examples from Ring House’s 2022 Music Festival, which featured 53 independent living residents collaborating with three professional musicians, we will highlight the health benefits of active, rather than passive, engagement with the arts. An informal survey conducted at the Festival finale indicated that 95% of the community found the three day program energizing and strengthened their sense of belonging. Lauren Latessa, Iris Music Project Executive Director, Brenda Rice, Vice President of Operations, Charles E. Smith Life Communities and Gabe Pinkney, Lifestyle and Leisure Manager, Ring House, discuss how the unique relationship between their organizations created the 2022 Music Festival and provide a framework for other communities interested in long-term partnerships with professional musicians.

Strengthening IPC Regulations and Antibiotic Surveillance through Post-Acute Analytics
Some 40-75% of prescribed antibiotics in SNFs may be unnecessary or inappropriate, causing risk of serious infections, increased adverse drug events and drug interactions, and even death. There’s also the threat of antibiotic-resistant infections, which afflict 2.8 million Americans annually. We know that monitoring and tracking antibiotic use is an arduous and time consuming process, but vital if we want to improve outcomes and eliminate needless side effects. The right technology can help by identifying the early warning signs of infection and highlighting the need for possible testing to determine the appropriate drug interventions. Operators can also leverage live data analysis to meet state and federal requirements, prepare for regulatory reporting and onsite surveys, and conduct infection surveillance.

The Benefits of Music as an Evidence-Based Non-Pharmaceutical Intervention for Those with Alzheimer’s, Dementia, and Other Neuro-cognitive Disorders
While music is a powerful evidence-based intervention, it continues to be underutilized and generically applied. According to Alzheimer’s Disease International, the number of people with dementia will nearly double every 20 years, reaching 78 million in 2030. Therefore, the need for quality memory care, effective nonpharmaceutical therapeutic interventions, and strategies for communities and families to provide meaningful engagement will also continue to grow. This presentation will discuss how neuroscience demonstrates the unique therapeutic benefits of music and its effects on multiple centers in the brain: stimulating long-term memory, promoting vocal fluency, stirring emotions, and stimulating motor control. Music has been shown to help reduce depression, anxiety, and agitation in those with dementia, as well as to help manage challenging behaviors and reduce the need for antipsychotics. Music also facilitates an individual’s connection with family members and caregivers and greatly adds to quality of life, leading to more positive outcomes.

The Trouble with Transfers: Developing Safe and Effective Strategies for Resident Transitions
The process for transfers between hospital, post-acute, and aging services settings can be complex and often creates multiple opportunities for failure. These risks include faulty handoffs or a setting that does not meet the resident’s care needs, resulting in harm. Operational, reputational and financial risks to the organization are added concerns. Assessment of current practices and development of improved procedures will help prevent harm, promote compliance with requirements and save valuable staff time. The presenters will discuss identification and mitigation of risks and offer important information to assist organizations in evaluating their transfer processes. (9/27 only).
